Working across health and social care NHS Stockport CCG will aim to:
- Reduce the number of people with long term conditions needing to be admitted to hospital.
- Increase independence for people living with a Long Term Condition.
- Support more people to die at home when they want to.
- Expand primary and community mental health provision.
- Improve care for people living with dementia and their carers.
