Mastercall Healthcare – Pathfinder in Nursing Homes
We have now commissioned Mastercall Healthcare to provide an ‘in hours’ triage and visiting service for patients in nursing/care homes with ‘urgent’ but non ‘life threatening’ conditions. This is part of our work to support admissions avoidance and appropriate utilisation of urgent and emergency services.
Five nursing homes across Stockport have been selected for this pilot scheme. They are:
- Appleton Manor
- Bamford Grange Nursing Home
- Hilltop Court
- Marple Dale Hall Care Home
- Newlands Nursing Home
Patients from these homes can be referred to this Mastercall service between 10am and 6pm Monday to Sunday.
The Mastercall GP will respond within 15 minutes by telephone and undertake a face-to-face consultation within two hours.
GPs should be aware that if they have patients who reside at any of these homes that they may be referred into the service if they require urgent treatment between the hours of 10am and 6pm.
GP practices will be informed by the Master Call GP on Adastra one hour after the close of the consultation.
