The Medicines Optimisation team is responsible for the clinical and cost effective use of medicines. The team works with practices to review drugs, and with pharmacies to encourage participation in patients’ medication reviews.

Medicines co-ordinators were introduced into GP practices in 2008/9 and most practices now have a Practice Based Medicines Coordinator (PBMC) as part of their team employed directly by the practice.
The team works with secondary care (Hospital) colleagues to agree product choices and treatment guidance. They also support other health provider services and the Local Authority in the use of medicines.
