Rapid Response is for patients who are not in need of an acute medical bed but might be experiencing one of the following:
- Loss of mobility
- Reduced carer support
- Sudden loss of function
- Discharged from ED or hospital ward but struggling to cope at home
- Deterioration of condition and feel unable to cope anymore at home
The service provides time limited care, treatment and support to patients who have an acute health condition in their own home. The aim is to aid their recovery and prevent hospital admission.
Patients need to be over 18, have a Stockport GP and need to live in Stockport (paying Stockport Council tax).
The service runs 7 days per week. Between the hours of 9 till 5pm it will be operated across the four localities. From 5 till 8.30pm it will be run borough wide through the Intermediate Care Team.
The maximum response time between referral and contact with the patients is 2 hours.
Excluded are:
- Service users in acute mental health crisis
- Service users needing an acute medical bed (medically unstable)
- Patients needing a fast-track into the District Nurse service/End of life care
GP referral:
- The GP is clinically responsible for an actual clinical judgement that the patient does not need an acute hospital bed and would benefit from a rapid response
- The GP will need to provide an up to date picture of the patient’s condition, brief history needs, access and safety at home, support that is already in place and urgency
- The patient will remain in care of GP when staying at home
