What is NHS Continuing Healthcare?

NHS Continuing Healthcare in the name given to a package of services which is arranged and funded by the NHS for people outside hospital with ongoing health needs.  Continuing Healthcare can be provided in any setting, in a care home, or at home.

If the person is in their own home, this means that the NHS will pay for healthcare (eg services from a community nurse or specialist therapist and personal care (eg help with bathing, dressing and laundry).  In a care home, the NHS pays for the care home fees.

Anyone assessed as requiring a certain level of care need can get NHS Continuing Healthcare.  If the person’s overall care needs show that your primary need is health, the person should qualify for NHS Continuing Healthcare.

The primary health need is assessed by looking at all of the person’s care needs and relating them to four key indicators:

  • Nature– the type of condition or treatment required and its quality and quantity
  • Complexity– symptoms that interact, making them difficult to manage or control.
  • Intensity – one or more needs which are so severe that they require regular interventions
  • Unpredictability – unexpected changes in condition that are difficult to manage and present a risk to you or to others.

Nurse Liaison Role

Purpose

NHS Stockport CCG commissions with all nursing homes in the Stockport area and the Continuing Healthcare Team will offer a Liaison Role with these homes.

Each home will have either a Lead Nurse or Nurse Assessor responsible to the home.

The purpose of this role is to offer advice and guidance to the nursing homes in relation to people’s needs, particularly supporting the homes to facilitate the National Framework for NHS Continuing Healthcare and NHS-funded Nursing Care, making sure the nursing homes have all the relevant information to ensure an accurate delivery of the Continuing Healthcare process.

The nurse will also be available to review people’s admissions and update NHS Stockport data.

It is important to visit weekly and build up a working relationship with the home.

Key Points

  • One nurse is to be responsible for each home, visiting on a fortnightly basis. (approximately ½ hourly visits)
  • Liaison with Home Managers and key workers to establish who the Continuing Healthcare Services support.
  • General discussion about people and to address any concerns, particularly if funded by Continuing Healthcare Services.
  • Advise the nursing homes of any updates on the National Framework and have discussion around the process within NHS Stockport CCG Continuing Healthcare Services.
  • To ensure the people are supported and their needs are being met (not to review – just be aware).
  • Offer appropriate training as requested.
  • Support SMBC with safeguarding issues and share all intelligence with Contract and Monitoring department.
  • Inform Administrative Team of all changing needs for the individuals, change in packages and new admissions.
  • Keep a written report on a weekly basis as to the visit made using the appropriate pro-forma.
  • Case notes to be kept up-to-date when specifics are discussed about an individual.
